How to Fill Out a Claim Form

First, fill in the form with your name exactly as it appears on your order.

Be sure to include the order/invoice number and the name of the product you are making a claim for.

The product name must match the one on the invoice. Briefly describe the reason for your claim and, if you are requesting an exchange, the name of the product you would like in exchange for the delivered item.

Also provide the account number where we should refund your payment. Remember to sign the form at the end. The bottom section of the claim form will be completed by a member of the claims department.
